The time listed above for these appointments is the amount of time you will spend with Dr. Shadid at each appointment. The appointment fees include the additional 30-60 minutes of work outside of appointment time for each patient/appointment it can take provider to score assessments, review specialty laboratory evaluations (from Genova Diagnostics, Genomind, ZRT Laboratories), prescribe medications, order lab-work, maintain and review medical record reports, coordination of care with other providers, responding to voicemail and email questions/concerns between appointments, and prior authorization requests with your insurance provider. This is different from other providers who typically charge additional fees and payments for these services outside of the appointment time.

Insurance Reimbursement

Dr. Shadid does not accept any insurance plans and is considered an out of network provider with all insurance companies. Shadid Integrative Psychiatry will not directly bill your insurance company for payment of services, but SIP will provide a receipt or invoice for you to submit directly for reimbursement from your insurance company. Please be aware that some of the services provided may be "non-covered" services and considered not medically necessary under your insurance plan. Insurance reimbursement depends on individual patients' policy terms, and Dr. Shadid is not responsible for ensuring insurance reimbursements.


The patient is responsible for payment in full, regardless of your insurance company's final determination of coverage. Please check with your insurance company before making an appointment in regards to your mental health benefits. Dr. Shadid does not accept Medicaid and/or Medicare. All patients who receive Medicaid/Medicare insurance must sign a federally mandated agreement in order to receive services at Shadid Integrative Psychiatry, which states that patients understand they are responsible for paying the full fee of appointments and will be ineligible to receive Medicaid/Medicare reimbursement for services at SIP.

Financial Policy

For established patients, full payment of the appointment is due at time of service. The full appointment fee will be charged for late-cancellation appointments (cancelled with less than 2 business days notification), or missed/no-show appointments. A credit card is required to keep on file for each patient in the event of a missed, no-show or late-cancellation of appointment, and this sensitive credit card information is kept confidential via a encrypted, HIPAA compliant patient portal program was patients are established as patients at Shadid Integrative Psychiatry.


For new patients, 50% of the new patient appointment fee will be collected when Dr. Shadid schedules your first appointment. The remaining balance will be charged on the date of service for the new patient appointment. Appointments cancelled with at least 3 business days notice will receive a full refund of the prepayment. The pre-payment will not be refunded for cancelled appointments with less than 3 business days notice or missed/no-show appointments. New patients must pay with credit card for the first intake/consultation appointment.


Office Policy and Prescription Policy

Dr. Shadid's office hours are Monday through Thursday 9:30 am-5:30 pm. Please ensure that all non-emergency clinical matters, including prescription refill requests, are handled during office hours of Monday-Thursday 9:30 am-5:30 pm. You may contact Dr. Shadid via email at nicole@drshadid.com, phone/voice message at 303-656-9215, fax to 303-459-7827, or via the HIPAA compliant patient portal system once patients are established at Shadid Integrative Psychiatry.


Dr. Shadid checks her voice message and email regularly and will make every attempt to return non-emergency/non-urgent matters within 24 hours, not including weekends or holidays. Non-emergency or non-urgent messages left through voicemail and/or email after office hours may not be returned until the following business day. SIP does not provide on-call 24 hour / 7 days a week crisis management services. Messages will be checked after office hours, but if your clinical matter is an emergency or urgent matter that requires medical and/or psychiatric intervention, please call 911 or proceed to the nearest emergency room.


For established patients needing a medication refill, please allow two business days for a prescription refill to be processed. Please have your pharmacy fax a medication refill request to 303-459-7827. All prescriptions will be refilled electronically during office hours. For refills of psychiatric medications, patients must be seen for follow up appointments at least every 3 months fin order for psychiatric medications to be refilled.


Patients not seen within the appropriate follow up time of three months will not be able to refill psychiatric medications. Please be sure to schedule your medication appointment before running out of medications. Patients who request their medications be refilled on an urgent basis during non-office hours of nights or weekends, will be charged an additional fee.
