Dr. Shadid checks her voicemail/email regularly and will make every attempt to return non-emergency/non-urgent matters within 24 hours. Non-emergency messages left after office hours will not be returned until the following business day. You may contact Dr. Shadid via email at office@drshadid.com, phone/voice message at 303-656-9215, or fax to 303-459-7827. Messages will be checked after office hours, but if your clinical matter is an emergency or urgent matter that requires medical and/or psychiatric intervention, please call 911 or proceed to the nearest emergency room.
New Patient Intake Consultation Appointment
This is a 3-hour Intake Appointment: $1,500
Patients can expect two hours face to face with Dr. Shadid for this new patient consultation. Dr. Shadid will spend an additional 3 hours reviewing your intake paperwork, providing a personalized treatment plan for you, as well as provide you an Intake Summary Report. This report is about 8-10 pages, is provided to you shortly after your intake appointment, and is included in the New Patient fee. Patients will spend about 30 minutes with Dr. Shadid's office manager answering any questions, scheduling your appointment, and reviewing paperwork for this appointment.
Integrative and Functional Neuropsychiatry Medicine Follow-Up Appointments
These are 45 minute consultation appointments: $500
Because Dr. Shadid is triple-board certified in Integrative Psychiatry, Integrative & Holistic Medicine, and Functional Medicine, the follow-up appointments may include a variety of consultation focuses for personalized and precision medicine practices, and will depend on the individual patient's needs.

Dr. Shadid has 45 - 120 minute consultation appointments for the above requests:
Some patients will need to schedule longer appointments to ensure adequate time for psychotherapy services, explanation of specialty laboratory evaluation, and discussion of all treatment options. Dr. Shadid charges $150 for every additional 15 minutes in session. Follow-up appointments every 1-3 months are typical in Dr. Shadid's practice. Appointments must be either in-office or via the HIPAA-compliant telepsychiatry service provided (phone appointments not available).
Psychedelic Medicine and Ketamine-Assisted Psychotherapy (KAP) Appointments
These are 90-120 minute appointments: $950-$1200
The use of ketamine therapy for mental health treatment is a novel psychiatric treatment, with initial studies showing benefit for Depression, Bipolar Depression, Suicidal Ideations, Anxiety Disorders, Post-Traumatic Stress Disorder, and Pain Disorders. Ketamine Treatments can be provided in either Intramuscular (IM), Sublingual/Oral (SL), or Intranasal (IN) Forms. Dr. Shadid is a certified Ketamine Provider in Psychedelic Medicine, and incorporate treatments with Ketamine-Assisted Psychotherapy (KAP) for the best patient results and outcomes.
In order to optimize quality patient care, Dr. Shadid has opted out of insurance networks, Medicaid, and Medicare, and is considered an "out of network" provider for all insurance plans. Shadid Integrative Psychiatry will provide an invoice for you (with the required procedure and diagnostic codes) to submit directly for reimbursement from your insurance company, so that you can receive all the benefits that you deserve as a policyholder. Patients are responsible for the entirety of your bill at the time of service, regardless of your insurance company's final determination of coverage. Insurance reimbursement depends on the patients' policy terms, so it is recommended to contact your insurance company regarding exclusions or other questions you may have regarding your insurance policy's mental health benefits.
For established patients, full payment of the appointment is due at time of service. The full appointment fee will be charged for late-cancellation appointments (cancelled with less than 2 business days notification), or missed and no-show appointments. A credit card is required to keep on file for each patient in the event of a missed appointments. For new patients, the appointment fee will be collected when the 3-hour New Patient/Consultation Appointment is scheduled. New patients who need to cancel their appointment will receive a full refund if appointments are cancelled at least 3 business days before the scheduled appointment. Dr. Shadid charges her appointment fee rates for any additional paperwork requests, such as filling out disability forms, treatment summary requests, or physician documentation for academic assistance letters or work purposes, etc.
Prescription refill requests are processed during office hours of Monday - Thursday 9:30am - 4:30pm. Refill requests will not be processed on nights, holidays or weekends. For refills of psychiatric medications, all patients must be seen for follow up appointment at least every 3 months. Patients not seen within the appropriate follow up time of three months will not be able to refill psychiatric medications. Patients will be provided enough medications to get to their next appointment. It is the patient's responsibility to schedule their follow up appointment before they run out of psychiatric medications.
