Type Length Fee
New Patient Intake/Consultation Appointment 90-120 minutes $850
Integrative Psychiatry Follow Up Appointment 30 minutes $300
Integrative Psychiatry Follow Up Appointment 60 minutes $500

Dr. Shadid offers three follow-up Integrative Psychiatry appointments: 30, 60, and 90-minute appointments. Patients scheduling any Psychotherapy Services or Specialty Laboratory Evaluations (such as Hormone, Genetic, Nutrition, Neurotransmitter, Autoimmune, Gastrointestinal Health, etc), will need to schedule either the 60 or 90-minute appointments to ensure adequate time for psychotherapy services, explanation of specialty laboratory evaluations, and discussion of all treatment options.

Because Dr. Shadid practices Integrative and Holistic Psychiatry, in conjunction with Functional Medicine for evaluation and treatment of mental health and psychiatric symptoms, most of the follow up appointments will be scheduled as 60 or 90 minute appointments to ensure adequate time to discuss multiple topics of health and wellbeing. The 30-minute appointments are reserved for medication management only, when patients are stable on their medications and needing a quicker medication refill appointment.

60 or 90-minute appointments can include:

1) Psychotherapy Appointments with or without Medication Management

2) Specialty Laboratory Evaluations - Hormone Testing, Micronutrient Testing, Neurotransmitter Testing, Genetic Testing, etc

3) Alternative and Complementary Treatment - Stress Management Techniques, Yoga Therapy, Meditation/Mindfulness, Breathwork

4) Functional Medicine Psychiatric Evaluations - Inflammation, Immune, Hormone, Nutritional or Gastrointestinal (GI) Assessments

New Patients may go to the New Patient tab for further information regarding the New Patient Appointment and the application process.

Insurance Reimbursement

In order to optimize quality patient care, Dr. Shadid has opted out of insurance networks, Medicaid, and Medicare. She is considered an "out of network" provider for all insurance plans, including Medicaid and Medicare. Shadid Integrative Psychiatry will not directly bill your insurance company for payment of services, but SIP will provide an invoice for you (with the required procedure and diagnostic codes) to submit directly for reimbursement from your insurance company, so that you can receive all the benefits that you deserve as a policyholder. Due to "exclusions" that may be part of your insurance policy, certain treatment services or laboratory evaluations may not be covered. Regardless of such exclusions, all services rendered by Dr. Shadid are considered medically or psychiatrically necessary. Patients are responsible for the entirety of your bill at the time of service, regardless of your insurance company's final determination of coverage. Insurance reimbursement depends on individual patients' policy terms, and Dr. Shadid is not responsible for ensuring insurance reimbursements. Please contact your insurance company regarding exclusions or other questions you may have regarding your insurance policy's mental health benefits and "going out of network".

Financial Policy

For established patients, full payment of the appointment is due at time of service. The full appointment fee will be charged for late-cancellation appointments (cancelled with less than 2 business days notification), or missed and no-show appointments. A credit card is required to keep on file for each patient in the event of a missed, no-show or late-cancellation of appointment. For new patients, 50% of the new patient appointment fee will be collected when Dr. Shadid schedules your first appointment. The remaining balance will be charged 3 business days before the new patient appointment, according to her Cancellation Policy. Appointments cancelled with at least 3 business days notice will receive a full refund of the prepayment, but the pre-payment will not be refunded for cancelled appointments with less than 3 days notice or missed and no-show appointments. Dr. Shadid charges her appointment fee rates for any additional paperwork requests, such as filling out disability forms, treatment summary requests for coordination of care with other providers, or physician documentation for academic assistance letters or work purposes, etc.

Prescription Policy

Prescription refill requests are processed during office hours of Monday - Thursday 9:30am - 5:30pm. For established patients needing a medication refill, please have your pharmacy fax a medication refill request to # (303) 459-7827, or the pharmacy may request medication refills electronically. Please allow two business days for a prescription refill to be processed. For refills of psychiatric medications, all patients must be seen for a follow up appointment at least every 3 months (quarterly for the year) in order for Dr. Shadid to continue prescribing psychiatric medications. Patients not seen within the appropriate follow up time of three months will not be able to refill psychiatric medications. Patients will be provided enough medications to get to their next appointment. Please be sure to schedule your medication appointment before running out of psychiatric medications.
